Required Forms

  • Parent/Guardian Consent for Digital Resources 
    • Teachers will use a variety of digital resources to help students learn during the 2023-24 school year. These digital resources have been checked and approved by Fairfax County Public Schools. Please provide your consent for your student to use these resources through the Parent Digital Consent System.
       
  • Student Rights and Responsibilities Acknowledgment Form:
    • Starting in the 2021-22 school year, parents/guardians with a ParentVUE account can download and digitally sign the SR&R in ParentVUE. Every parent/guardian with a ParentVUE account, regardless of relationship to the student, will see the SR&R upon login to PVUE until the SR&R acknowledgement has been completed and/or until the acknowledgement made inactive by IT at the client’s request.  If the parent chooses, he/she/they may “skip” the SR&R acknowledgement by clicking on “Remind me later!”. The next time the parent/guardian logs in the SR&R acknowledgement will appear again.
    • The parent/guardian will NOT be required to click on the pdf document link to download/view the SR&R packet before the parent/guardian will be able to digitally sign and acknowledge the SR&R. The name in the digital signature box must match the parent first and last name in SIS exactly before the signature will be accepted. Schools will be provided with a resource sheet to assist parents / guardians in using Parent VUE to endorse the Parent / Guardian Signature Sheet.
    • While use of ParentVUE  to submit the SR&R signature sheet is preferred, parents / guardians are not required to use ParentVUE to submit the SR&R Parent / Guardian Signature Sheet. In cases where ParentVUE is not used, parents / guardians should download the signature sheet (pg. 3) in the SR&R from the FCPS public site. If a parent / guardian needs assistance accessing the signature sheet, they may contact their child’s school and ask that the school provide the form.

 

  • WeCare:The application weCare has new features which help manage parent contact information and their student’s current health conditions that may impact their school day. This information must be updated at the beginning of each school year, and may be updated whenever there is a change.

    •   In order to log into Parent View it is necessary to have an active SIS Parent Account.  Once the SIS Parent Account is created the same log in credentials will also work for Schoology: Parent View.
    • PARENT/GUARDIAN CONTACT INFORMATION: Please review the information to be sure the email addresses and phone numbers are correct and list at least 2 emergency contacts that you permit to pick up your student in the event of an emergency. Completing this portion creates the Emergency Care form which is automatically forwarded to the registrar.
    • CURRENT HEALTH CONDITIONS AND PHYSICIAN INFORMATION: Completing the health portion in weCare creates the Health Information form which is automatically forwarded to the Public Health Nurse.
